COVID – 19 Policy for Visitors & Volunteers

Due to the COVID-19 outbreak Care One Health Training Institute expects all visitors or volunteers that experience symptoms associated with the COVID-19, to call the school at 919-773-7165 and stay home. If the symptoms present while at school, visitors or volunteers are expected to report it to the administrative staff immediately. These symptoms are: Fever of 100.4 F or 38 C, cough, shortness of breath, or difficulty breathing, fatigue, headache, muscle or body aches, sore throat, loss of taste or smell, congestion or runny nose, nausea or vomiting, and diarrhea.

Any visitors or volunteers that develop signs and symptoms of a respiratory infection while on site, should:

Immediately stop, put on a facemask, and self-isolate at home;
Inform the facility’s infection preventionist, and include information on individuals, equipment, and locations the person came in contact with; and
Contact and follow the local health department’s recommendations for next steps (e.g., testing, locations for treatment).

Visitors or volunteers should follow Infection Control Guidelines when on-site.

Visitors or volunteers are required to answer the questions below related to the COVID-19 and if one or more of the questions are answered “YES” they will not be allowed to enter the building.

1. Have you traveled to a designated “hot spot” (Hotspot according to CDC COVID data tracker-https://www.cdc.gov/coronavirus/2019-ncov/travelers/map-and-travel-notices.html) in the last 14 days? *YesNo

2. Have you come into close contact with someone who has a laboratory confirmed COVID-19 diagnosis within the last 14 days? *YesNo

3. Do you have a fever (greater than 100.4F or 38C) OR symptoms of lower respiratory illness such as cough, shortness of breath, difficulty breathing, fatigue, headache, muscle or body aches, sore throat, loss of taste or smell, congestion or runny nose, nausea or vomiting, and diarrhea? *YesNo

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

Care One Health Training Institute has put in place preventative measures to reduce the spread of COVID-19; however, Care One cannot guarantee that you will not become infected with COVID-19. Further, attending Classes could increase your risk of contracting COVID-19

I agreeBy signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending Care One and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Care One may result from the actions, omissions, or negligence of myself and others, including, but not limited to, other program participants and their families I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at Care One or participation in Care One programming (“Claims”). On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless Care One, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Care One, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Care One programs.
